Looking for a home for my parents

I live in Montreal-Canada and am looking for a long term care center for my mother and father that are both not well.

My mother had a stroke six years ago and is since then disabled physically with health complications because of diebeties.

My father is for the past two years diagnosed with Dementia, but is fully physically independant.

I need a place that takes care of both problems in the central Israel area, (Petah tikva, kfar Saba...) where they medically taken care of and also have a good quality of life.

I also plan on moving close to where they will be.

I was told that such places do exist in Israel but for some reason, I am not able to find any on the internet.
